Form A and B should be completed by individuals who want the check results sent to them. However, the Form B check result is notarized. Form C and D are for employment purposes. Form C check result is sent to the applicant’s address, while the check result for Form D is sent to the employer. A copy of the applicant’s government-issued photo identification is required to process each check. After completing the required form, an applicant must schedule a fingerprinting appointment by calling IDEMIA at 1-(877) 503-5981 or visiting New Jersey Fingerprinting Services website.

Each fingerprint-based request costs $42.80. MasterCard, American Express, Visa and Prepaid debit cards are accepted for payment. The completed fingerprint form and a copy of the applicant’s government-issued photo identification are required to process the live scan fingerprinting service.

To obtain a name-based criminal history record check, complete and submit an Online Name Check Criminal History Record Request. Each check costs $20, payable by MasterCard, Visa, Discover, American Express credit or debit cards. A valid Email address must be provided on the online form.
